  Complaints must be submitted in writing to the Arizona State Board of Chiropractic Examiners via their online form, email, or mail.

• Necessary Information: Name, address, the name of the chiropractor, specific details, and treatment dates.
• Process: The board reviews complaints, conducts investigations, and can issue disciplinary actions ranging from letters of concern to license suspension or revocation. 

Patients can also check for past disciplinary actions through the Board's online Public Records Request system.
